Fragilité et déclins des personnes âgées à domicile : une méthode co-construite de suivi de proximité

Abstract : As the proportion of the elderly population is constantly increasing, the scientific community is emphasizing the need for early diagnosis of frailty in order to prevent loss of autonomy. Frailty is a state of increased vulnerability to stress factors that can lead to functional disability. This state may be reversible. While frailty was first defined and identified based on its physical implications, more recent research highlights its multidimensional characteristics. Indeed, frailty impact psychological and social dimensions too. It is therefore important to assess this impact in order to provide support. There is a particular interest in setting up home support services. In order to improve the management of frailty, we propose to generalize its identification at home. Thus, this work aims at building and validating a multidimensional frailty identification method based on the knowledge of home care workers. With the intention of making this identification earlier, we propose to evaluate the possibility of integrating an automated data collection.The first step in this work was to compile a list of indicators of frailty. This list was proposed to a group of experts in order to evaluate the relevance of the choices made.The experts were also questioned on the interest of partial automation of a data collection for the identification of frailty. At the same time, a review of the literature highlighted the indicators whose evaluation could be automated. The identification tool and method were then co-constructed with the home care workers. The co-construction of the method can facilitate its deployment and its appropriateness to the problems of the professionals who work in the home. Finally, the statistical analyses carried out showed the factorial validity of the tool constructed.
