Résumé

Introduction: only few pediatric studies report the pharmacokinetics and anti-CMV effect of ganciclovir (GCV), especially in patients undergoing hematopoietic stem cell transplantation (HSCT). Some exposure targets have been proposed in adults but remains unexplored in children. The main objectives of this study were to: i) model GCV concentrations in children, ii) describe blood CMV loads, using nonlinear mixed effects model, iii) quantify the therapeutic effects of antiviral drugs administered, iv) explain interindividual variability by covariates (e.g., age, weight, creatinine clearance, etc...). Material and methods: a total of 105 children on GCV or valganciclovir, followed up at Necker hospital, were included in the PK study. Among them, children undergoing HSCT were selected to investigate the antiviral effect. Blood samples were collected for routine laboratory examination. CMV loads were assessed weekly. All the data were analyzed using non-linear modeling with mixed effects. Results: a two-compartment model, with first-order absorption for valganciclovir and first-order elimination, best described the data. Bodyweight (BW) effect was represented with an allometric model. Estimated glomerular filtration rate (eGFR) and critically ill status were significantly associated with GCV elimination. Recommended doses were adequate for prophylactic treatment. However, current dosing algorithm might be associated with a high risk of under-exposure for curative therapy. Viral loads were investigated using a semi-physiologic and mechanistic model. GCV produced a 95% inhibition on CMV production constant. FSC exerted little additional antiviral effect when associated with GCV. GCV exposure impact on viral dynamics was not significant within the studied range. Lymphocyte counts had a significant effect on viral elimination. Conclusion: a high rate of pediatric exposures below adult therapeutic targets, following current GCV recommended doses, might suggest underdosing in children, especially for curative treatment. Dosing adjustment in the pediatric population should be reevaluated with respect to body weight and renal function. In this study on the concentration-effect link in HSCT children, CMV clearance was observed despite GCV AUC0-24h <40 mcg*h/mL.
Introduction : la pharmacocinétique/pharmacodynamie du ganciclovir (GCV) est peu rapportée dans les études pédiatriques, en particulier chez les patients ayant une transplantation de cellules souches hématopoïétiques (CSH). Certaines cibles d'exposition ont été proposées chez l'adulte mais restent discutables chez l'enfant. Ainsi, nous avons effectué cette étude pour l'objective de : i) modéliser les concentrations de GCV chez les enfants, ii) décrire les charges virales sanguines de CMV, en utilisant un modèle non linéaire à effets mixtes, iii) quantifier les effets thérapeutiques des médicaments antiviraux administrés, iv) expliquer la variabilité interindividuelle par des covariables, telles que l'âge, le poids, la clairance de la créatinine. Matériel et méthodes : au total, 105 enfants sous GCV ou valganciclovir, suivis à l'hôpital Necker, ont été inclus dans l'étude PK. Parmi eux, les enfants transplantés de CSH ont été sélectionnés pour analyser l'effet antiviral. Des prélèvements sanguins ont été réalisés dans le cadre du bilan d'examens de routine. Les charges virales ont été évaluées toutes les semaines ou plus fréquemment en fonction du contexte clinique. Toutes les données ont été analysées à l'aide d'une modélisation non linéaire à effets mixtes. Résultats : un modèle à deux compartiments, avec une absorption du premier ordre pour le valganciclovir et une élimination du premier ordre, décrit le mieux les données. L'effet du poids corporel (BW) a été représenté avec un modèle allométrique. Le taux de filtration glomérulaire estimé (eGFR) et l'état critique étaient significativement associés à l'élimination du GCV. Les doses recommandées étaient adéquates pour le traitement prophylactique. Cependant, pour le traitement curatif, l'algorithme de posologie actuellement utilisé pourrait être associé à un risque élevé de sous-exposition. Les charges virales ont été évaluées à l'aide d'un modèle semi-physiologique et mécanistique. Le GCV a produit une activité inhibitrice de 95 % sur la production de CMV. Le FSC a exercé peu d'effet antiviral supplémentaire lorsqu'il est associé au GCV. L'exposition au GCV n'a pas conduit à un effet significatif sur les charges virales dans l'intervalle d'AUC étudié. L'effet du nombre de lymphocytes a été trouvé sur l'élimination virale. Conclusion : un taux élevé d'enfants sous-exposés, comparé aux cibles thérapeutiques chez l'adulte, suivant les doses actuellement recommandées du GCV, suggèrent un sous-dosage chez les enfants, en particulier pour le traitement curatif. L'adaptation posologique dans la population pédiatrique devra être réévaluée en fonction du poids corporel et de la fonction rénale. Dans l'étude sur la relation concentration-effet chez l'enfant transplanté de CSH, la clairance virale du CMV a été constatée malgré une AUC0-24h <40 mcg*h/mL du GCV.

Trong so do, nhung tre duoc ghep te bao goc tao mau duoc dua vao phan tich tac dung khang virus. Viec lay mau duoc tien hanh theo phac do xet nghiem mau thuong qui. Tai luong virus duoc xet nghiem hang tuan hoac thuong xuyen hon tuy theo tinh trang benh. Cac du lieu duoc phan tich bang phuong phap mo hinh hoa phi tuyen tinh voi hieu ung hon hop. Ket qua: du lieu duoc dong hoc duoc mo ta gan dung nhat bang mo hinh 2 ngan, hap thu bac 1 doi voi valganciclovir duong uong va thai tru bac 1. Anh huong cua can nang duoc bieu dien bang mo hinh di trac hoc. Muc loc cau than va tinh trang benh nang co anh huong co y nghia thong ke len su thai tru thuoc. Lieu dung khuyen cao hien nay phu hop cho dieu tri du phong. Tuy nhien, trong dieu tri dac hieu, che do lieu hien hanh co the co nguy co cao khong du hieu qua. GCV co tac dung uc che 95% su sinh truong cua virus. Foscarnet khong co nhieu tac dung khang virus bo sung khi dung cung GCV. Muc do GCV trong mau khong anh huong co y nghia thong ke len su thai tru virus, xet trong khoang AUC nghien cuu. So luong te bao lympho co anh huong len su thai tru virus. Ket luan: doi chieu voi muc tieu dieu tri o nguoi lon, voi che do lieu khuyen cao hien hanh, so tre co muc GCV thap hon muc tieu chiem ti le cao, goi y rang viec dieu tri cho tre em chua du hieu qua, dac biet la dieu tri dac hieu.
