Déficits fonctionnels sémiotiques dans la maladie d'Alzheimer. : Remise en question des schémas descriptifs actuels et propositions thérapeutiques

Abstract : Alzheimer's disease is common. Mainly, but not exclusively, this disease concerns elderly and very old people. Disease is associated with memory disorders and Impairment of judgment. Semiotics allows to approach the reasons of the loss of the sense in the physical surrounding and human environmentof the patient. Semiotics points and qualifies efforts made by patients to remain in a signifying world for them. Semiotics put in perspective new ways to establish patient care. The semiotics’ approach in this thesis was conducted on one hand from two studies, one on the narrativity from a text memorizedby demented patients, the second on the dissolution of the limits in patient's relationship with their community caregiver, and on other the other hand, from clinical cases analysis. New pathways have been described in the natural history of demented patients according to the evolution of the tensivespace and according to their forms of life. Rupture in the continuity of the course of the illness appears, precipitating an irreversible situation. Prevention of this break has been proposed. The alterations of the coupling between the presence of the world in patients ‘psychic intimacy and their presences ofself in the world have consequences on the mean ing involved both in their actions and in their interpersonal relations. The reciprocal human relation and using world of the patients ‘imaginary are therapeutic resources. A new therapeutic resource has been proposed: the analogon.
