Asthme, tabagisme et déficits émotionnels

Abstract : Backgrounds and objectives: Due to their high morbidity and mortality rates, asthma and smoking constitute a major public health issue. This study aims at understanding the relations between medical and psychological factors such as alexithymia, depression, anxiety and emotions among smoker and non-smoker asthmatic patients.Methods: 70 asthmatic patients were included in this study (17% female, 82% male, mean age 54 years +/- 13.7). Structured interviews were conducted to assess a series of validated questionnaires regarding: 1/ asthma evaluation with the Asthma Control Test and Questionnaire (ACT and ACQ), 2/ medication compliance with the Medication Adherence Rating Scale (MARS-5), 3/ quality of life (Asthma Quality of Life Questionnaire: mini-AQLQ), 4/ alexithymia (the Twenty-Item Toronto Alexithymia Scale: TAS-20), 5/ depression and anxiety (Hospital Anxiety and Depression Scale: HADS), 6/ emotions (Diener scale) and 7/ smoking behaviours (Smoking Behaviours Questionnaire: SBQ and Fagreström test). In addition, two projective tests were assessed during the interview: the tree-drawing test and the draw-a-person test. This mixed-method study uses an integration of quantitative and qualitative data analysis.Results: On one hand, this study highlights relation between asthma severity, its control and quality of life. On the other hand, it suggests a connection between alexithymia, anxiety, depression and negative emotions in asthmatic patients. This study also shows that medical and psychological factors are worse when patients are alexithymic, depressive, anxious or have partially or uncontrolled asthma.Significance: This highlights suggest clinical implications for professionals working with asthmatic patients. Indeed, the detection of the most vulnerable patients by identifying and evaluating alexithymia, anxiety and depression can provide an optimization of the patient’s follow up; and offer a suitable psychological support using cognitive and behavioral techniques or even propose an artistic mediation linking somatic and emotional aspects like theatre or singing.
