CARACTERISATION MOLECULAIRE DE LA VIRULENCE ET DE LA RESISTANCE AUX ANTIBIOTIQUES DE HELICOBACTER PYLORI DANS DES BIOPSIES GASTRIQUES HUMAINES EN CÔTE D'IVOIRE

Abstract : Helicobacter pylori infection (H. pylori) affects more than 50% of world's population and causes many severe gastroduodenal diseases such as peptic ulcers, MALT lymphoma and gastric cancer. Problem of H. pylori infection lies in resistance of bacterium to antibiotics used and its evolution towards cancer. This work focused on molecular characterization of H. pylori virulence and antibiotic resistance in chronic gastroduodenal infections. Polymerase Chain Reaction (PCR) methods and genetic sequencing allowed this study to be conducted. Study of genetic diversity of H. pylori virulence made it possible to describe a genotype profile s1a + / m1a + / cagA + / oipA + and tipA + involved in chronicity of infection and to suggest that presence of multiple genotypes would be necessary to colonization and persistence of H. pylori in the gastric mucosa. The results of search for antibiotic resistance genes used for treatment of H. pylori in Côte d'Ivoire, showed a high level of Amoxicillin resistance (58.2%) and presence of point mutations in genes conferring resistance to antibiotics used for treatment. However, no cross-resistance to clarithromycin and metronidazole combination was identified in this study. This could guide treatment protocol of H. pylori to priority combination of these two molecules (clarithromycin and metronidazole) in the Ivorian hospital context. Patients with H. pylori strains with antibiotic resistance genes predominantly had lesions characterized by inflammation and redness in gastric mucosa associated with presence and persistence of H. pylori. These results support the need for an evolution for current therapeutic protocol in Côte d'Ivoire. Results obtained in this study are essential. They serve as basis for better characterization of H. pylori infection and for effective management of emergence of gastroduodenal diseases related to this bacterium in Côte d'Ivoire.
