Crack : substância, corpos, dispositivo e vulnerabilidades. A psicanálise e a prática clínico-institucional com usuários de crack

Abstract : The research draws from my clinical-institutional experience in the field of drugs and it is centered on a reflection about the practice of psychoanalysis with crack users in situation of precariousness, vulnerability, and exclusion. My main goal is to examine the subjective processes related to the problematic uses of crack, in their psychical, corporeal, sociopolitical, and cultural dimensions. I propose the notion of "crack dispositif" in order to examine and reflect upon the network of discourses, interpretations, interventions, strategies, attention, and care, which takes shape around the crack phenomenon in contemporary Brazil. Such notion allowed me to call attention to the biopolitical field in which the crack problem is embedded, and the interweaving and mutually supportive relations between power and knowledge about the drug. By articulating the sociopolitical field to the subjective field, the notion of "crack dispositif " also revealed the processes of subjectification and desubjectification there implied. The analysis of the potential subjective effects and incidences resulting from the situations of exclusion and multiple vulnerabilities that characterize the majority of crack users in Brazil allowed for the identification of its tight connection to crack abuse. Exclusion and vulnerability are deeply articulated with long-term use of crack (and its related effects), as well as with the constitution of a subjective conjuncture in which the use of crack can be interpreted as a "survival strategy" in the face of "extreme situations of subjectivity". The construction and analysis of a clinical case indicated that the patient´s toxic strategy is tightly linked to his context of precariousness and marginalization. The case made explicit the effects of the "crack dispositif " in the singularity dimension of the subject and its subjective positioning. The analysis of the clinical case also contributed to a reflection about corporality and crack abuse. This reflection emphasized that sociopolitical processes, power dynamics, hegemonic discourses, inequalities and marginalizations permeate and condition the very materiality of bodies and their drives. A focus on the body conjugates and allows for the emergence of the different dimensions intertwined in the complex phenomenon of crack: the potency of the chemical substance and its effects over body and subjectivity; the subjective and singular issues; the dynamics of drives; the sociopolitical determinations and processes; the situations of vulnerability and exclusion; the strategies and interventions mobilized; and the dominant discourses. At last, I call attention to the fundamental specificities of the psychoanalytical listening, clinical positioning, and interventions in the clinic of crack users.
