Résumé

The seasonal characteristics of the epidemics of influenza are admitted and proved. They are mainly studied by laboratories of public health. The seasonal constituent of these epidemics underlies the influence of climatic parameters, beforehand isolated by the climatological method called "types of weather". A precice analysis of the climato-meteorological conditions concluded to highlight specific "types of weather" observed before the epidemics releases. If the climate plays an important role, other factors like promiscuity or transport networks would explain certainly the regional disparities observed in terms of flu-like incidence. After a discussion about the factors playing potentially a role in the increase of the vulnerability, we ended in a mapping of this vulnerability. Secondly, we integrated some of these factors within the framework of a model to predict the rate of flu-like incidence: this try of modelling seems encouraging. Finally, by crossing the map of exposure at "risky types of weather" with that of the global vulnerability, it becomes possible to map the flu-like risk in metropolitan France. The geographical conception of the risk becomes from then additional of that used in epidemiology.
Les épidémies de grippe, dont le caractère saisonnier est désormais admis et prouvé, sont majoritairement étudiées par des équipes de recherche en santé publique. La composante saisonnière de ces épidémies sous-tend l'influence de paramètres climatiques, préalablement isolés par la méthode dite des types de temps. Une analyse fine des conditions climato-météorologiques a aboutis à la mise en lumière de " types de temps à grippe ". Si le climat joue un rôle important, d'autres facteurs liés à la promiscuité ou les réseaux de transports expliqueraient certainement les disparités régionales observées en termes d'incidence grippale. Après une discussion des facteurs jouant potentiellement un rôle dans l'augmentation de la vulnérabilité, nous avons aboutis à une cartographie de cette vulnérabilité. Dans un second temps, nous avons intégré certains de ces facteurs dans le cadre d'un modèle afin de prédire le taux d'incidence grippale : cet essai de modélisation semble concluant. Finalement, en croisant la carte d'exposition aux types de temps à risque avec celle de la vulnérabilité globale, il devient possible de cartographier le risque grippal en France métropolitaine. La conception géographique du risque devient dès lors complémentaire de celle employée en épidémiologie.
