Indices de la réponse au remplissage vasculaire chez les patients ventilés par pression positive

Abstract : The present PhD work studied the clinical utility and accuracy of some indices which were tested as bedside indicators of preload reserve and fluid responsiveness in hypotensive patients under positive pressure ventilation. In the major studies, static indexes contrary to dynamic indexes are not valuable tools to identify hypotension related to a decrease in preload and fluid responsiveness.

In the first part of this work, we demonstrate that in a majority of critically ill patients, dynamic indexes are better indices than static indexes to predict fluid responsiveness.

During the second part, we display in animal studies that abdominal hypertension and protective ventilation with low tidal volume in animals with ARDS and low pulmonary compliance, may disturb the accuracy of dynamic indexes to predict fluid responsiveness
