Les revenus des médecins généralistes - trois études microéconométriques

Abstract : The aim of this thesis is the analysis of French General Practitioners earnings. Three microeconometric studies are developed using longitudinal data.

A first chapter deals with the impact of the medical population's regulation on the career of the general practitioners. We show that the intergenerational inequity is huge and that the demographic control of educated practitioners plays a key role on the doctors fees. Practitioners installed in the 1980s suffer from both baby-boom effect and high numerus clausus rule: They get the lowest income. On the contrary, the decrease in the numerus clausus level improved the financial situation of the installed cohorts from the middle of the 1990s.

A second chapter looks at a remarkable point in the general practitioners' population: the existence of a large minority of low incomes physicians. We note that 5 to 7 % of the general practitioners have an income lower that 1.5 net SMIC (French minimum wage). We show that these low incomes result from a greater preference for leisure. Their very poor activity do not reflect a downgrading of the practitioners' profession, but rather one of its advantages, as a private profession: practitioners may choose to work less.

A third chapter assesses the positioning of general practitioners incomes versus those of managers. Comparing the career value of general practitioners and managers, we show that the total sum of discounted wages of practitioners is much higher than this of managers, even when the opportunity cost of the studies are taken into account. The competitive examination required for the admission to medical schools results in establishing an allowance for the whole career. This increased over the years with a growing examination difficulty.
