Le simulateur d'accouchement BirthSIM : un outil complet pour la formation sans risque en obstétrique

Abstract : Because complications related to the use of obstetrical instruments comes from a training problem of the future obstetricians and midwives, we propose the childbirth simulator BirthSIM as a tool to acquire a first experience in obstetrics. Indeed, a simulator offers the medical team a risk-free training to complete the traditional training carried out in the delivery ward.

The BirthSIM simulator is composed of anthropomorphic manikins with the anatomical landmarks used in the delivery ward. To replicate the involved forces, a pneumatic actuator is controlled by a servovalve with position and effort control algorithms to simulate
different deliveries. To evaluate the obstetric gesture, it is discomposed into two steps: the forceps placement and the extraction gesture. To analyze the placement, the forceps are instrumented with position sensors with the ability to measure the six degrees of freedom. Methods to
analyze the gesture are then developed to objectively evaluate the operator repeatability, their error rate and their similarity compared to a reference gesture. Concerning the extraction gesture it is recorded with a force sensor. All parameters related to this extraction (maximal force, work, etc.) are gathered in an evaluation function to calculate a performance index which allows to evaluate objectively the gesture.

Finally, a visualization interface offers a risk free training method to operators to learn the obstetric gesture. The evaluation of the gestures is carried out using the developed methods which allow to quantify the skill progression of the obstetricians trained on the
BirthSIM simulator. The results confirm and complete the disparate results found in the literature and attest to the interest of a simulator.
