Développement d'un appareil d'auto-rééducation par mobilisation assistée et évaluation de son intérêt thérapeutique chez des personnes immobilisées en fauteuil

Abstract : Chronic sitting immobilisation is armful for health and leads to deconditioning. People dependent on a wheelchair are prone to sever musculoskeletal and cardiopulmonary disorders. Blood stasis, due to the sitting position, increases the risk of peripheral vascular disease. Home-based medical device for wheelchair bound subject rehabilitation are very uncommon. The first part of this thesis has been carried out in collaboration with DCO Engineering. This preliminary research based on a kinematics analysis and a motion simulation of cycling has led to the elaboration of a cycling ergometer (R-go dom) The second part of this work is related to the evaluation of the passive cycling therapeutic interest on the peripheral vascular system in people with paraplegia (n=15). Acute passive leg cycle exercise increases the blood flow in the femoral artery (+30%) and decreases the vascular resistance. Furthermore 6 week of passive leg cycle exercise training (n=14) may lead to chronic arterial adaptation. Hence we have demonstrated that this cycling device can be used at home by paraplegic subject in self-sufficiently. As pyramidal lesion is common in this immobilized population, we have measured with the pendulum test the impact of passive cycling exercise on muscular spasticity. ln conclusion passive cycling has a therapeutic interest for immobilised person. The spreading of this rehabilitation technique should be encouraged for Self rehabilitation in addition to the usual care.
