Étude longitudinale de la qualité de vie et des stratégies d'ajustement des patientes avec un cancer du sein et de leur « accompagnant-référent »

Abstract : Researches concerning social support in Health Psychology have put forward the importance of the “natural” social environment throughout life and more particularly at stressful moments. In this theoretical approach, the social resources belong to the transactional processes modifying the experience of stress, as well as coping which allows to be adjusted with the stressors. In parallel, the legislative evolutions validate today the role of the “caregivers” recognized like “informal carers” in the context of disease or dependence (law of February 11, 2005). From this double point of view, this doctoral work aims to integrate the disease Object, the sick Subject and the Alter “caregiver” for a better apprehension to transactional processes concerned in the context of breast cancer at the initial phase. For this purpose, 100 patients accompanied by a significant other (N = 100) were met at three times (after the disease's notification, at the end of treatments and 6 months after the end of treatments) in order to collect data on their appraisal of the situation, their coping strategies, their social interactions and their quality of life. The results show that (1) the transactional processes are dependent on contextual factors; (2) the sense of duty, the norm of reciprocity and especially the profane representations on the importance of social ties justify the behaviour of informal carers; (3) the association of the various strategies of coping seems to be an interesting track to study the relations between coping and quality of life. Ultimately, these results put forward relevant indicators for study transactional processes in the context of breast cancer, but especially, invite to a reflection on the fundamental role of the appraisal of the situation and on emotions in the processes in implementation.
