L'Imagerie Interventionnelle par Résonance Magnétique: Application en thermothérapie laser interstitielle pour le traitement des tumeurs hépatiques.

Abstract : Fight against cancer witnessed the rise of new patient care enhancing technologies. Laser ablation is one of the most recent in situ destruction techniques for tumors. An optical fiber has therefore got to be introduced into the core of the tumor by inserting a sheath through skin. This laser diode sends photon so that the tumor can be burned locally without opening the patient, thus considerably reducing the average hospitalization stay. The gesture that has to be performed is however so complex that it dims the great advantages of this therapy. For guidance during each stage, surgeons could use an MRI scanner.
This thesis, financed by the Nord Pas de Calais Region and the CHRU from 2005 to 2007, aims at providing tools for interventional MRI during laser ablation. This project will thus not only propose a tool to prepare this highly complex intervention (three-dimensional visualization of patient's tumors, computation of the best trajectory to avoid perforation of vital anatomical structures ...), but will also provide peroperative assisting tools (sheath positioning assisted by stereovision on early centimeter, evaluation of thermal damage of the processed area).
During this first step, we turned our attention on hepatic tumors, which remain a challenge, but the system we proposed is adaptable to any abdominal tumor.
