Caractère biphasique de l'hypoxie intermittente : mécanismes cardioprotecteurs induits par une exposition aiguë et conséquences inflammatoires d'une exposition chronique.

Abstract : Intermittent hypoxia (IH) is one of the major component of obstructive sleep apnea (OSA) syndrome. This disease which affects 5% of general population, is characterized by reccurent upper airway collapses during sleep and is associated with an increase in cardiovascular risk.
These studies were performed in order to characterize the cardiac effects of an acute exposure and the inflammatory effects of a chronic exposure to IH, in rats and mice respectively.
A 4 h IH with 10% O2 induces delayed cardioprotection illustrated by an improvement in post-ischemic functional recovery and a reduction in infarct size in isolated rat heart. This protective mechanism is called preconditioning (PC). IH-induced PC is triggered by p38MAP kinase and Erk1/2 and mediated by PKC, iNOS and mitochondrial KATP channel. A better understanding of the protective mechanisms implicated in the IH-induced PC should allow identification of targets in order to prevent cardiovascular diseases.
Chronic IH with 5% O2 induces an increase in the proliferative capacity of spleen derived T-cells, an upregulation of pro inflammatory molecules levels, an increase in leukocyte rolling on the endothelial cell layer. These data demonstrate an inflammatory response in mice exposed to chronic IH which
may allow a better understanding of the cardiovascular complications of OSA syndrome, specially atherosclerosis developed by those patients.
