Evaluation ultrasonore de l'os cortical par transmission axiale : modélisation et expérimentation in vitro et in vivo.

Abstract : The so-called ultrasonic axial transmission technique allows to assess cortical bone status, based on velocity measurements. This technique is investigated in this work through numerical simulations of the involved propagation on one hand, and through an experimental work on the other hand. A finite differences code has been implemented to model wave propagation in cortical bone, by simulating 3D elastic propagation in anisotropic and heterogeneous media. With this code, the nature of the ultrasonic wave propagating along cortical shells is investigated, as a function of bone parameters such as geometry (cortical thickness, curvature) or intracortical microporosity. In parallel, a prototype has been developed and modified to suit clinical measurements requirements. In particular, new probes have been designed and validated, allowing reproducible measurements on cortical bone both in vivo and in vitro, based on a bidirectional transmission principle. An in vitro study has been carried out on 50 human radius, combining ultrasonic measurements to X rays measurements (conventional medical scanner and Synchtrotron microtomography). For the first time, we evaluated respective effects of materiel bone mineral density and microporosity on axial transmission velocity measurements. A clinical study is currently carried on, with 150 measured subjects so far. In vivo reproducibility has been shown to be appropriate, thanks to bidirectional transmission. We present preliminary results concerning age effect.
