Défis à la mise en oeuvre de la prévention de la transmission mère-enfant du VIH en Afrique australe. Le cas d'un district rural du Zimbabwe

Abstract : In August 2001, pilot Prevention of Mother-To-Child Transmission (PMTCT) services were introduced in a rural mission hospital, Murambinda Mission Hospital (MMH) (Buhera district, Manicaland province, Zimbabwe).
These PMTCT interventions were the first routine HIV prevention activities within the hospital. Buhera district has provided the setting for four quantitative and qualitative operational research studies conducted between June 2001 and August 2004. Two main research issues were addressed: 1) the progress in understanding and use of PTMCT services and HIV prevention services in general; 2) the infant feeding practices adopted in the context of HIV and well as the availability of related mother and infant follow-up and support services after delivery.
This research has highlighted the importance to broaden the scope of PMTCT interventions outside maternity services and to take into account the socio-cultural and economic aspects of HIV infection. Further investments in health promotion strategies and behaviour change initiatives, facilitating modifications in social norms regarding infant feeding and HIV infection, are urgently needed, especially in the domain of maternal and child health in African countries with elevated HIV prevalance.
