Identification des axes d’amélioration de la chaîne des secours sur piste : étude qualitative sur le ressenti des médecins de montagne et des pisteurs secouristes

Abstract : Objective: To explore the expectations of both mountain doctors and ski-patrollers towards the organization of the rescue chain on the ski slopes, in order to identify the possibilities of improvement. Method: A qualitative study through multiprofessional focus groups, gathering physicians and ski-patrollers, in Northern Alps ski resorts which counted more than 200 rescues per year. Data collection was done using an interview guide. The verbatims were studied according to a thematic phenomenological analysis. Results: The lack of interaction between ski-patrollers and physicians appeared to be the main issue. They wished to set up communication spaces in order to promote interprofessionalism. The management of pain on the ski slopes was also a significant difficulty. The solutions were to protocolize the administration of analgesics by the ski-patrollers, and to set up a nurse-patroller who could pre-medicalize the victims before evacuation. Finally, interventions on the ski slopes of the mountain doctors were the main alternative when the helicopter could not intervene. A reflection on these conditions of intervention, very different from one ski resort to another, must be led to allow standardization of practices. Conclusion: Interprofessionalism and the involvement of mountain doctors and ski-patrollers seem to be a prequisite to initiating change. The collaboration of the emergency medical services centers, the ski patrol and community services is also necessary for the accomplishment of a common project, in the priority interest of victims.
