Abstract : Objective. To evaluate if using menstrual cup (cup) is an intrauterine device (IUD) expulsion risk factor. Material and method. Epidemiologic prospective multicentric pilot study with a cohort follow-up. Patients attended medical centers in the French Alps and were followed-up during a time frame going from June 2016 to August 2017. The study of one patient starts the day of IUD set up by a doctor, noting down whether they are using a cup or not, and called back one year after. The Kaplan-Meier plot was constructed to estimate the occurrence of IUD expulsion in the case of users of cups, versus non users, adjusted on a cox model. Results. 216 schedules were filed, 207 patients were included and 22 patients (10.6%) were lost of sight, 75 patients were cup users at the beginning and 61 patients at the end. 12 (6.5%) patients expelled their IUD, 10.5% in users versus 4.2% in non-users (p=0.09). During menstruations, 75% of patients using cups expelled their IUD versus zero patient amongst non-users (p=0.055). The Kaplan-Meier plot found no difference with using the cup at the inclusion time, even with adjustment (HR=2.57, [0.81-8.1]). The secondary analysis found a statistic difference in effective users of the CM at one year (HR=4.3, [1.3-14.2]). Discussion. This pilot study suggests that there is a higher IUD expulsion risk when using menstrual cups, especially during menstruations. However, this pilot study has some bias due to low sampling leading to a lack of statistical strength while providing up to date and innovative inputs for a wider scale study.
